Form N-4, Monthly Report on Naturalization Papers

Form N-4

The purpose of Form N-4 is for the clerk of any court which conducts naturalization activities under Section 339 of the Immigration and Nationality Act (8 United States Code 1450) and 8 CFR 339. The information contained in Form N-4 is used by the United States Citizenship and Immigration Services to finalize the record process with regard to individual's who have been naturalized. Form N-4 is also used to determine payment to the courts as mandated by law.

Form N-4 lists the date of the naturalization ceremony conducted, the court list number, the location of the ceremony, the number of persons administered the oath of citizenship, and the number of voided certificates. Form N-4 should be submitted monthly by the Clerk of the Court. USCIS headquarters is the appropriate recipient of Form N-4. Copies and attachments evidencing the details of the naturalization ceremonies conducted during the relevant month must be attached and submitted concurrently with Form N-4.

The authority of the USCIS to collect the information reported by the Clerk of the Court on Form N-4 is based on the authority contained in Section 339 of the Immigration and Nationality Act with the specific purpose to determine payments to the courts.
