Form G-1145, E-Notification of Application/Petition Acceptance
The purpose of Form G-1145 is to allow the applicant or petitioner who can file their application or petition at one of the three USCIS Lockbox facilities, to receive electronic notification that their application or petition has been accepted.
A completed and signed Form G-1145 should be clipped to the first page of the application or petition.
