Form G-1041A, Genealogy Records Request
Form G-1041A is used to request copies of United States Citizenship and Immigration Services historical documents. Form G-1041A should be used by anyone who is a: researcher requesting a copy of a USCIS historical record by file number; or anyone who has received file numbers resulting form the Genealogy Index Search Request (Form G-1041); or a researcher seeking records for genealogy or family history purposes or heir location, and who can provide precise historical record file numbers and can identify the immigrant by name and/or other information.
For records of naturalization prior to September 27, 1906, Form G-1041A should not be used. Neither should Form G-1041A be used to research historical databases for sea, land or air passenger manifest lists recording arrivals prior to December 1982. Form G-1041A is not the proper form for a request for return of original documents. (Form G-884 is the proper form for such a request.)
Form G-1041A requires exact historical USCIS file numbers. No record pursuant to a File G-1041A can be retrieved without a file number. If the file number is unknown, the researcher must submit a Form G-1041 to request the file number of the USCIS historical document.
