Form DS-261, Choice of Address and Agent (NEW: Limited Use)
Form DS-261 is required for certain applicants at designed United States Embassies and Consulates after the applicant has been notified by the National Visa Center. The Form DS-261 is a web-based electronic form and will eventually replace the current Form DS-3032 Choice of Address and Agent. Not all immigrant visa applicants are required to use the new Form DS-261 at the present time.
Form DS-261 is used to collect information necessary to process immigrant visas.
The applicant must use Form DS-260 and Form DS-261, Choice of Address and Agent if:
1. The petition was approved in the United States;
2. The application is being process by the National Visa Center and
3. The application is family based (Form I-130) , employment based (Form I-140) or for certain immigrants filing Form I-360 petition, or for an immigrant entrepreneur (Form I-526).
Form DS-260 and Form DS-261 are limited to the following countries where interviews will be conducted in the following cities: Form DS-261 visa applicants will be interviewed at any United States Embassy or Consulate in Mexico; or after October 2010, other visa applicants will be interviewed at any United States Embassy or Consulate listed below:
1. Canada-Montreal
2. Greece, Athens
3. Jordan, Amman
4. Iraq, Baghdad
5. Peru, Lima
