Form DS-1648, New Application for A, G, or NATO Visa (For Applicants Applying in the U.S. Only)

Form DS-1648 is used for applying for an A, G, or NATO visa. The Form DS-1648 requires the applicant to submit a photograph of the applicant which must comply with certain standards. The Form DS-1648 applicant must provide the location of the United States Embassy or Consulate for where he or she is submitting the Form DS-1648. Form DC-1648 requires biographical information about the applicant as well as his or her nationality. Passport information must be provided on Form DC-1648 including all surnames listed on the passport. If the applicant's passport does not indicate a given name, the Form DS-1648 applicant should enter “FNU” in the area pertaining to given names.

Form DS-1648 requires the applicant to identify any family members who are permanent residents or citizens of the United States. Additional the Form DS-1648 applicant must indicate whether he or she has ever been arrested or convicted of any offense or crime, even if later granted a pardon, amnesty or other similar legal action. The Form DS-1648 applicant must also state whether or not whether or not he or she has ever unlawfully sold or distributed an unlawful substance or engaged in prostitution.

Form DS-1648 also requires the applicant to state whether or not he or she has ever been denied admission to the United States or has been the subject of a deportation hearing.
