Form DS-156K, Non-immigrant Fiance(e) Visa Application

Form DS-156K

Form DS-156K is used for a fiance or fiancee of a United States citizen to apply for a visa to enter the United States. Form DS-156K must be filed along with Form DS-156, Non-immigrant Visa Application, in order for the fiance(e) visa application to be complete.

The alien fiance(e) must proved biographical information on Form DS-156K as well as the names and place of birth of any unmarried children who are under twenty-one years of age. Information about any previous marriages must be provided on Form DS-156K.

The following documents are required to be attached to the Form DC-156K in order to apply:

1. the applicant's birth certificates 2. a marriage certificate if any 3. evidence of engagement to the fiance(e) 4. evidence of financial support 5. divorce decree if any 6. death certificate of spouse if any 7. birth certificates of any children 8. police certificates

As indicated on Form DS-156K, all of the above listed documents are also required by USCIS when applying for adjustment of status to lawful permanent resident.

Furthermore, as indicated on Form DS-156K, if the applicant is currently married, he or she is not eligible for a Form DS-156K Non-immigrant Fiance(e) Visa and the application will be denied.
