Form DS-0234, Special Immigrant Visa Biodata Form

Form DS-0234

Form DS-0234 is used for special immigrant visa applicants who may qualify for resettlement assitance from the Department of State. Form DS-0234 must be completed for each family member of the principal Form DS-0234 applicant.

The Form DS-0234 applicant must provide information as to the number of people in his family that will be traveling with the principal applicant. If the applicant is not the principal applicant, the person completing the Form DS-0234 application must indicate his or her relationship to the principal Form DS-0234 applicant.

Information as to eduction, skills training, and employment are required in Form DS-0234. Information pertaining to the applicant's language skills including native language spoken, other languages, and English language ability are also required in Form DS-0234.

The applicant may indicate his or her choice of cities to be relocated in the United States. Pursuant to Form DS-0234 consideration is given to cities where the applicant may have family members living in the United States. Those family members of the Form DS-0234 applicant must be identified by name, address, telephone number and relationship to the applicant. Also, any family members who are being processed with Form DS-0234 applications must be identified in Form DS-0234 for cross-referencing purposes.
